Assembled Products offers
repair service for all versions of the Martin controller board at a fixed
rate plus shipping. Use this web based return authorization request for more
accurate processing.
1) Simply fill out this form and press the
submit button at the bottom of the form. This will notify us that your
controller is coming. Please fill out the form as completely as possible so
that we can process your request as quickly as possible!
2) Ship your controller board to our facility.
We suggest that you obtain a tracking number for your shipment and/or
insurance on the shipment in the event that the package is lost in transit.

3) We will call you and/or e-mail you as soon
as your control board is repaired to obtain credit card payment information.
Your control board will then be shipped back, typically by UPS, with a
tracking number.
